IMHO, anything larger than an 18 looks retarded. At that point, you start to raise the center of gravity, loose agility, and it looks like a 4x4. Like Kenny said, keep the 18's and lower the car. You'll gain a lot more. You'll get good looks and better handling.

Spacers go between the rim and the hub to make them stick out further.

Depends on the rims you have and what you are doing with them. I have seen factory 17's used with 20mm spacers in the rear and it filled in the rear wells very nicely.
