Clutch wear - that's the problem. Starting in 2nd means you've got to start out with higher RPMs to get enough torque to get moving AND you've got to have the clutch partially engaged longer too. Unless of course you're dumping the clutch in 2nd, which is about 1000 times worse than smooth starting in 2nd.
If you're in traffic and can keep from coming to a complete stop, stay in 2nd all the time. If you have to stop and go, it's better to use 1st for starting, but excuseable to keep it in 2nd and feather s needed, just know that you are shortening your clutch life. If you're using 2nd to start as a regular practice in daily driving, you're just throwing away money 'cuz you're going to need a new clutch twice as fast as normal.
As far as skip shifting (2nd-4th or 5th, 3rd-6th, etc) no harm done. Get your rocks off on a hard 0-50mph, then settle in to a smooth cruise going direct to 5th. As long as you're doing a decent job of matching the revs when you shift, it doesn't matter what gear you choose.
