shifter boot/ exhasutmanifold flange gasket

OK two questions

1. where can I get a good shifter boot or make one the ones that the auto parts stores sell keep ripping.

2.   all the auto parts stores show I should have a two bolt exhaust manifold flange but mine has a three bolt donut type on it is this orignal or a modification.  any ideas on where to get a donut?
