KA24E timing chain tensioner problem

anyone know why the plug/cup on the end of the timing chain tensioner (KA24E) would come/pop out? It has caused timing chain noise and a loss of some oil pressure.........

Can I simply buy/find a new cup/plug and put it back in without pulling the whole front off to replace the tensioner ........... the engine sat for a long time and we ran sea foam in it a short time.

cant picture it right now but too little tension on the chain may have caused it. I would use the same piston if it wasnt damaged...you would have to pull the front cover to do this. this can be done without touching the head or the pan(other than taking out the 4 bolts on head and 2pan bolts and crank pulley.
