engine rattling

hey yall im new to the forum an owner of a 93 Nissan d21, its a good little truck but its got some small problems. first it misses at an idle an acts like its about to die, but if you throttle down it acts just fine. ive done changed plugs wires distributer cap an fuel filter.... second it has a pecking noise when u accelerate but if u take it out of gear an throttle up it makes no noise at all. I need some help trouble shooting this so I can get started on my new project, thank yall for your time
