Clutch won't disengage

Having trouble with clutch. When the pedal is pressed all the way down, the engine and trans are still connected (wheels spin and extremely difficult to switch gears). I recently swapped a 5 speed trans into the car so clutch, flywheel, clutch master cylinder, slave cylinder, and throwout bearing all new OEM replacement parts. I have tried to adjust the clutch and it seemed to help some but rod on the master cylinder was adjusted to the point where it was about to separate from the pedal. Slave cylinder moves when the pedal is depressed and system has been bled multiple times. Not sure if it's an issue with actual clutch components, I'm fairly sure everything was installed correctly but I could be wrong. Any advice would be appreciated.

Are you certain that your clutch fork didn't come loose from the throwout bearing?

Or the fork has dislodged from the pivot ball??
